Terms of Service

By clicking the Submit button, I agree to the terms of service.

This Agreement is entered into at 2219 W. Melinda Ln., Building A, Phoenix, AZ 85027 and KOWALSKI at it’s option, may bring suit in Maricopa County Superior Court or Northwest Justice Court.

OWNER may cancel this Agreement before the start of work and receive a full refund if KOWALSKI has incurred no costs on OWNER’S behalf.  OWNER will be responsible for any costs incurred prior to cancellation including Travel Time & Trip Charges.  KOWALSKI may cancel this agreement, without cause, before the start of work.

Labor will charged “Portal to Portal” & includes travel time and supply restocking for all service work.

Holiday Rates apply on the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day Following Thanksgiving and Christmas Day.

CONSTRUCTION SERVICE:

Labor will be charged according to the function performed. Standard Time is charged for regular hours between M-F 7 A.M. – 5 P.M.Overtime rates of 1 ½ times Standard Time will be charged for hours outside regular hours and on Saturday. Overtime rates of 2 times Standard Time will be charged on Sundays and Holidays. All travel time, material and equipment procurement and delivery time will be charged to the job. A minimum of 3 hours per person will be charged regardless of the actual time spent on the job. When same day service is requested and provided it will be billed as overtime.

Material will be charged according to the actual cost of material secured for the job.

Equipment will be charged according to prevailing rental rates. Owner must provide at least 24 hours notice to arrange pick up of equipment left on site. Minimum charges may apply.

Subcontract work will be billed as charged by the subcontractor.

Overhead of ten percent (10%) of the subtotal will be added to each invoice.

Profit of ten percent (10%) of the sum of the subtotal and overhead will be added to each invoice.

ELECTRICAL SERVICE:

Standard Rate – Monday-Friday 6 A.M. – 3 P.M. @ $90.00 per hour (Preferred Customers) / @ $95.00 per hour (Standard Customers)

Overtime Rate – 1 ½ times Standard Rate will be charged for hours outside regular hours and on Saturday.

Double Overtime Rate – 2 times Standard Rate will be charged on Sundays and Holidays.

Emergency Rate – @ $135.00 per hour (Preferred Customers) / @ $142.50 per hour (Standard Customers)

A $25.00 Trip Charge will be added to each Service Call.
